Church volunteer sign-up form: a free template and what to ask
The rota is only as good as the information behind it. A short sign-up form, collected once or twice a year, saves a lot of back-and-forth.
What to ask
- Name and contact details, and how they prefer to be contacted.
- Which teams they would like to join, and any they would rather not.
- Skills or experience, such as sound desk, reading aloud or food hygiene.
- How often they are happy to serve: weekly, twice a month, monthly.
- Away dates they already know about.
- Permission to share their name and number with the rest of the team.

If a role involves working with children or vulnerable adults, follow your church’s safeguarding policy for that role before adding anyone to the rota.
Running a sign-up Sunday
- Put forms and pens on a table by the door, with someone there to answer questions.
- Mention it in the notices for two weeks running.
- Offer a “try it once” option. People are more likely to sign up if it is not a lifelong commitment.
- Thank people personally when you add them to the rota.
